Job Summary
Conduct intensive, ambulatory care and crisis management for youth aged 14 to 24 with complex psychiatric and social challenges. Act as a casemanager responsible for your own caseload, including leading treatment planning, conducting intake interviews, and performing outreach to clients in their homes, schools, or neighborhoods. Collaborate with a multidisciplinary team to support recovery across housing, employment, and social relationships while maintaining contact with parents and external networks. Work within the FACT Jeugdteam West in Amsterdam, where you will frequently visit clients in the Jordaan, Baarsjes, and Nieuw-West areas. This role involves flexible caseload adjustments based on client needs and requires a BIG-registered nurse with at least two years of experience in GGZ or ambulatory care.
Required Qualifications
- HBO-opleiding Verpleegkunde
- BIG-registratie
- Minimaal 2 jaar werkervaring in het verpleegkundig vakgebied
- Ervaring met jeugd, de EPA-doelgroep en het voeren van systeemgesprekken
- Het overleggen van een Verklaring Omtrent Gedrag (VOG)
