Job Summary
Conduct outreach psychiatric care for adults with serious mental health conditions, including home visits and community-based interventions. Lead treatment planning, monitor medication effects and side effects, and perform both psychiatric and somatic assessments. Manage crisis situations, coordinate with general practitioners and hospitals, and actively contribute to team innovation and quality development. Collaborate intensively within a multidisciplinary FACT-team while maintaining regie over your own agenda.
