Job Summary
Conduct outreach psychiatric care for adults with severe mental disorders, managing treatment plans, prescribing medication, and monitoring clinical progress. Perform intakes, somatic examinations, and crisis interventions while collaborating within a multidisciplinary FACT team. Coordinate with general practitioners and hospitals to ensure continuity of care and contribute to team innovation and quality development. Maintain strict oversight of treatment safety and client well-being in community settings.
