Job Summary
Conduct comprehensive mobile vascular assessments for patients in skilled nursing facilities, rehabilitation centers, and outpatient locations, focusing on peripheral arterial disease, chronic limb-threatening ischemia, venous disease, and non-healing wounds. Evaluate wound characteristics, interpret diagnostic findings such as Doppler studies and ABI results, and coordinate care across multidisciplinary teams to prevent limb loss. Perform pre-procedural evaluations for vascular interventions, document clinical assessments in the EHR, and serve as a primary patient liaison for education and communication. Travel daily between multiple clinical sites while collaborating with specialists, physicians, and facility staff to ensure seamless diagnosis, treatment, and follow-up. Maintain compliance with regulatory and payer requirements while supporting limb preservation initiatives and quality improvement activities.
Required Qualifications
- Master's or Doctorate Degree in Nursing
- Current Nurse Practitioner license
- Current DEA registration
- Current BLS certification
- National Board Certification as Nurse Practitioner
- Valid driver's license with reliable transportation
- Strong knowledge of vascular disease processes, including Peripheral Arterial Disease (PAD), Chronic Limb-Threatening Ischemia (CLTI), venous insufficiency, diabetic wound management, and limb preservation/salvage principles
- Thorough understanding of vascular assessment techniques, wound care practices, vascular terminology, pharmacology, anticoagulation management, and infection recognition
- Knowledge of Medicare, CMS, and payer documentation standards, including medical necessity requirements, coding support, and electronic health record (EHR) documentation best practices
- Ability to perform comprehensive patient evaluations, interpret clinical findings, and identify patients requiring urgent vascular intervention
- Excellent clinical assessment, critical thinking, and problem-solving skills with the ability to make sound, independent clinical decisions
- Strong verbal and written communication skills, with the ability to effectively educate patients, families, and caregivers regarding vascular conditions and treatment plans
- Demonstrated ability to collaborate effectively with physicians, advanced practice providers, wound care specialists, and multidisciplinary healthcare teams
- Strong organizational, time-management, and care coordination skills, with the ability to manage multiple patients, priorities, and clinical locations in a fast-paced environment
- Ability to maintain accurate, timely, and compliant documentation while ensuring continuity of care across all treatment settings
- Ability to travel independently and drive daily between multiple clinical locations, including skilled nursing facilities, assisted living communities, rehabilitation centers, physician offices, and outpatient wound care centers
- Ability to stand, walk, bend, stoop, kneel, and move throughout clinical environments for extended periods while providing patient care
- Ability to lift, carry, transport, and operate portable medical and vascular diagnostic equipment weighing up to 25 pounds
- Ability to perform comprehensive lower-extremity vascular examinations, wound assessments, and other hands-on clinical evaluations requiring manual dexterity and visual acuity
- Ability to work autonomously in a mobile clinical setting, effectively managing patient care across multiple locations while maintaining productivity and clinical quality standards
- Ability to respond to changing schedules, travel demands, and patient care priorities in a fast-paced healthcare environment
- The position requires Covid vaccination
Desired Qualifications
- Preferred clinical experience in Vascular Medicine, Cardiology, Wound Care, Interventional Radiology, Interventional Cardiology, Vascular Surgery, Critical Care, or Internal Medicine, with demonstrated expertise in the assessment and management of complex vascular patients
- Preferred experience managing patients with Peripheral Arterial Disease (PAD), Chronic Limb-Threatening Ischemia (CLTI), Venous Disease, Diabetic Limb Preservation programs, and delivering care in mobile or community-based healthcare settings
- ACLS preferred
