Job Summary
Entry-level Value Based Care Specialist at eClinicalWorks. The role involves education, counseling, advocacy, and coordination of services for patients using the eClinicalWorks & healow product suite. The specialist acts as an extension of the patient’s caregiver or PCP to close gaps in care, delivering 1–2 virtual sessions per month and supporting programs such as CCM and RPM (as well as BHI and PCM). Responsibilities include patient interfaces via phone, email, and text; providing education and support for chronic illness management; contributing to reporting; utilizing care management plans within the product suite; ensuring CMS compliance; collaborating with care teams to assess patients and families; employing motivational interviewing to support self-management goals; coordinating care plans, services, and resources; documenting appropriately in the EHR/healow systems; and facilitating referrals and follow-up to improve outcomes. The ideal candidate should have experience with CCM/RPM programs, proficiency with EHR software, and a customer-service orientation, with the ability to work independently in a fast-paced environment. Required Qualifications
- An MA, CA, RN, NP, LPN, LVN, PA, CNA with at least 2 years' work experience in your role is mandatory
- Familiarity with Chronic Care Management, Remote Patient Monitoring or similar value-based care programs.
- 1 - 2 years of experience and proficiency working with Electronic Health Records (EHR) software as a user.
