Job Summary

Coordinate value-based care and population health initiatives by monitoring a patient panel, closing HEDIS care gaps, and collaborating with providers and health plans. Responsibilities include data collection and analysis to close care gaps, attestation of chronic conditions, participation in routine performance meetings with health plans, use of provider portals, and supporting all population health initiatives. Requires in-office presence in Scottsdale, AZ, with strong communication, organizational, and problem-solving abilities, and proficiency with electronic health records (eCW) and related software.

Required Qualifications

  • 1 year experience in Population Health or Medical Billing
  • Minimum 1.5 years experience in a Corporate / Health care Setting
  • MA experience highly preferred
  • Ability to work independently as well as in a Team
  • This role requires Full Time-In Office Presence
  • High level of professionalism
  • Excellent written and verbal communication skills
  • Excellent organizational and problem-solving abilities
  • Ability to work collaboratively with diverse teams and departments
  • Knowledge of healthcare regulations and compliance standards
  • Strong communication and interpersonal skills
  • Proficiency in electronic health records (EHR); eCW preferred, Microsoft products, and other relevant software.
