Job Summary
Manage treatment services for assigned caseloads by coordinating care, facilitating recovery groups, and overseeing patient transitions from admission to discharge. Provide weekly coordination of care sessions, document behavioral interventions, and address crisis situations while maintaining communication with counseling teams, post-treatment providers, and family members. Complete aftercare assessments, discharge plans, and necessary documentation for insurance and regulatory reports in compliance with state laws and accreditation standards.
Required Qualifications
- Bachelor's Degree in Behavioral Sciences such as Human Services, Social Work, Psychology or other related field
- Minimum of 2 years experience in a behavioral healthcare field and/or substance abuse recovery field
- Experience in behavior health treatment including, but not limited to, group facilitation, behavior intervention, community liaison, as well as case management for individuals diagnosed with a substance use and/or mental health disorder
- Knowledge of utilizing electronic health record software and documentation
- Knowledge of ASAM criteria
- Ability to read, write and interpret written information, type documentation, present information to groups and participate in meetings
- Working knowledge of Microsoft & Google Products
- Must be able to lift 15 pounds at a time
- State recognized (IC&RC) alcohol and drug counseling certification (or intern SUD counselor certification)
