Job Summary
Coordinate patient care by managing a caseload, conducting weekly coordination sessions, and overseeing LOC transitions and discharge planning. Document communication with counseling teams, business development reps, and external providers while facilitating life skills groups and monitoring community resources. Address behavioral concerns and crisis situations, ensuring all documentation adheres to state laws, accreditation standards, and HIPAA guidelines. Participate in interdisciplinary team meetings and required clinical supervision to maintain recovery stability for adults with addiction and co-occurring disorders.
Required Qualifications
- Bachelor's Degree in Behavioral Sciences such as Human Services, Social Work, Psychology or other related field
- Minimum of 2 years experience in a behavioral healthcare field and/or substance abuse recovery field
- Experience in behavior health treatment including, but not limited to, group facilitation, behavior intervention, community liaison, as well as case management for individuals diagnosed with a substance use and/or mental health disorder
- Knowledge of utilizing electronic health record software and documentation
- Knowledge of ASAM criteria
- Ability to read, write and interpret written information, type documentation, present information to groups and participate in meetings
- Working knowledge of Microsoft & Google Products
- State recognized (IC&RC) alcohol and drug counseling certification (or intern SUD counselor certification)
