Home Health Field LVN/LPN delivers nurse care in patients' homes under RN supervision. Responsibilities include assessing health status, administering medications, providing wound care, coordinating with physicians, therapists, and family, and developing/implementing individualized care plans to promote independence and improve well-being. Requires infection control, standard precautions, effective communication with patient/family and care team, and monitoring to ensure third-party payor requirements are met. Demonstrates clinical judgment, educates patients and caregivers on treatment protocols, and supports recovery and chronic disease management outside traditional clinical settings.
