Job Summary
Implement Individual Support Plans to provide monitoring, skill-building, and encouragement for daily living, community access, and housing stability. Assist consumers with emotional support, nutrition, budgeting, and personal care activities including feeding, toileting, and dressing. Provide transportation to appointments and errands while maintaining accurate service documentation and progress reports. Communicate with case managers, family members, and agencies to coordinate resources and report health or safety concerns. Lift and transfer adult consumers up to 50 pounds using proper safety procedures in community-based service locations. Maintain confidentiality and follow mandated reporting requirements while supporting consumers toward independence.
Required Qualifications
- High school diploma or GED
- Valid California driver license
- Reliable transportation
- Proof of automobile insurance
- Acceptable driving record
- Valid First Aid and CPR certification
- Ability to obtain First Aid and CPR certification within required timeline
- Ability to successfully complete required background clearance
- Ability to successfully complete fingerprinting
- Ability to successfully complete TB clearance
- Ability to successfully complete physical examination
- Ability to successfully complete other employment-related screenings
Desired Qualifications
- Minimum of six months of related experience working with individuals with disabilities
- College coursework in social welfare, sociology, psychology, education, social services, or a related field
