Job Summary
Provide all aspects of nursing care for the assigned home health patient case load, including completing assessments and tailored plans of care for both home health and hospice patients. Care for patients at all stages of life in their own homes while working as part of a multi-disciplinary team with a high level of autonomy. Complete documentation as assigned and utilize strong clinical support and educational resources provided. This full-time, travel contract role offers a guaranteed 40-hour work week from 8:00 am to 5:00 pm, Monday through Friday, with no weekends, holidays, or call. Candidates must hold an RN license for New Hampshire or Compact, BLS certification, and 1.5 years of current home health experience. Valid driver's license and auto insurance are required.
Required Qualifications
- Registered Nurse experience: 1.5 to 2 years
- Current experience Home Health RN experience: 1.5 years
- Valid Driver's License
- Personal Auto Insurance
- RN License for State of NH or Compact
Desired Qualifications
- Prior Hospice experience in addition to home health experience
- Prior Med Surg experience
