Job Summary
Home Therapy Registered Nurse trains patients and care partners in the patient’s home for dialysis, including peritoneal dialysis and hemodialysis modalities, with on-site clinic support and routine home visits. Responsibilities include evaluating home environments, documenting nursing services in the EMR, participating in infection control and emergency preparedness, coordinating supplies and orders, monitoring patient outcomes, and providing ongoing training and support to patients and care partners. Requires current RN license in state, relevant clinical experience in dialysis, CPR, English proficiency, and computer skills; orientation and competency for the modality are required.
Required Qualifications
- Current RN license in applicable state
- 12 months of clinical nursing experience in providing nursing care with at least 3 months in the modality or states with more stringent requirements (e.g., Texas)
- Experience in nursing care for patients on maintenance dialysis preferred
- CPR certification required prior to patient care assignment
- Ability to distinguish all primary colors
- Completion of Home Therapy Nurse Orientation program and annual competency for modality
- Proficiency in English (verbal and written)
- Basic computer skills; proficiency in USRC clinical applications within 90 days of hire
- Must meet applicable state practice requirements and any required background clearances/orientation
- Ability to meet minimum hiring standards for employment
