Job Summary
Provide and coordinate care for assigned patients in their homes, developing nursing care plans for Start of Care and admissions as assigned. Support patient recovery by educating families on disease management and ensuring physicians' verbal orders are transcribed appropriately. Conduct ongoing patient status assessments, document visit charting within 24 hours, and notify physicians of significant changes. Cover a 50-mile radius from the Everett office with days shifts (8:00 am – 5:00 pm), 40 hours weekly, including OASIS charting and case management oversight.
Required Qualifications
- Registered Nurse
- 2 years
- Recent/Current Home Health Nursing experience
- 1 year within the last 3 years
- OASIS experience
- Home Care Home Base (HCHB) charting
- Valid Driver's License
- Personal Auto Insurance
- ASN or an equivalent
- RN License for State of the state applying or Compact if applicable
- BLS Certification from AHA or American Red Cross
- Must be able to lift 50 lbs
- Must be available for weekend shifts
- Must be able to effectively lead and coach others
