Job Summary
Provide one-on-one in-home physical therapy services in patients' homes across the I-495 belt, following physician orders. Conduct initial evaluations, ongoing assessments, and develop individualized treatment plans to improve mobility, strength, balance, and overall quality of life. Deliver skilled therapeutic interventions, exercises, and patient/caregiver education; document progress per standards. Collaborate with nurses, physicians, and interdisciplinary teams to ensure coordinated care. Maintain a safe, professional therapy environment; travel within assigned service areas as needed. Requires MA PT license and accreditation; full-time schedule (40 hours/week).
Required Qualifications
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist (PT/RPT) license in Massachusetts
