Job Summary
Provide on-site and remote direct therapeutic services to pediatric and adult patients, including assessments, treatment plans, and discharge planning. Engage with staff and families to offer information, education, brief consultations, and referrals for mental health needs. Conduct rapid clinical assessments using standardized tools to identify patients requiring ongoing services and assist in navigating treatment. Maintain weekly therapy appointments and complete required documentation, including clinical assessments and treatment planning, within 48 hours of visits. Participate in clinical team meetings with Urgent Care and mental health staff while demonstrating deep knowledge of the intersection of physical and mental health issues.
Required Qualifications
- Must be independently licensed in state of practice
- Knowledge of medical terminology
- Understanding the roles of the healthcare team
- Exceptional interpersonal skills
- Deep understanding the psychosocial aspects of illness and health
- Experience of community facilities and resources for discharge planning
- Superb written and verbal communication skills
- Strong crisis intervention skills
- Excellent short-term counseling skills
