Job Summary
Provide pediatric and adult mental health therapy as part of a large multidisciplinary behavioral health team in an innovative pediatric-model primary/mental health care setting. Responsibilities include assessments, treatment planning, discharge planning, and providing on-site and remote therapeutic services to patients and families (ages 5–26 and adults). Conduct clinical assessments, utilize standardized tools (PHQ9, GAD7), maintain timely documentation, participate in clinical team meetings, and coordinate referrals to PM Mental Health services or other resources. Must be independently licensed and capable of making independent clinical decisions with guidance as needed. This role involves direct patient care in Urgent Care centers with available on-site locations listed (Riverdale, Mamaroneck, Forest Hills, Syosset, Selden) and hybrid work options.
Required Qualifications
- Must be independently licensed in state of practice
- Licensed clinical social worker (LMSW/LCSW) or licensed mental health counselor (LMHC)
- Knowledge of medical terminology
- Experience in pediatric and adult mental health
- Crisis intervention skills
- Excellent communication and documentation skills
- PHQ9, GAD7 usage in clinical practice
- Ability to provide on-site and remote therapy
- Team collaboration within a multidisciplinary behavioral health team
