Job Summary

Conduct head-to-toe assessments, triage urgent clinical situations, and manage wound care, IV therapy, and medication reconciliation for acute and post-acute clients in the home setting. Independently make real-time clinical decisions, perform comprehensive documentation, and monitor patient safety while adapting to varied home environments and dynamic visit schedules. Requires a current New Hampshire nursing license, one year of recent hands-on experience in acute or home health settings, and proficiency in point-of-care technology. Weekly pay ranges from $61.00 to $67.00 per point with flexible scheduling, PTO, and mileage reimbursement. Join a multi-disciplinary team delivering skilled nursing and rehabilitative care with full training and 24/7 clinical support.

Required Qualifications

  • A current nursing license in good standing in the state of New Hampshire
  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
  • At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings)

Desired Qualifications

  • Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition
  • Utilize your experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care)
  • Triage and manage urgent clinical situations independently in the home setting
  • Perform medication reconciliation, teaching, and safety monitoring
  • Perform comprehensive wound assessment, care, and documentation
  • Ability to work independently while knowing when to escalate concerns
  • Comfortable making real-time clinical decisions without immediate on-site support
