Job Summary
Conduct head-to-toe assessments, triage urgent clinical situations, and manage wound care, IV therapy, and medication reconciliation for acute and post-acute clients in the home setting. Independently make real-time clinical decisions, perform comprehensive documentation, and monitor patient safety while adapting to varied home environments and dynamic visit schedules. Requires a current New Hampshire nursing license, one year of recent hands-on experience in acute or home health settings, and proficiency in point-of-care technology. Weekly pay ranges from $61.00 to $67.00 per point with flexible scheduling, PTO, and mileage reimbursement. Required Qualifications
- A current nursing license in good standing in the state of New Hampshire
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings)
