Unit Ward Clerk/Health Information Management (HIM)

Job Summary
Organize and maintain clerical functions for nursing units by managing resident medical records, ensuring proper filing, and reviewing documents for completeness. Assemble charts for admissions, transfers, and discharges while handling requests in compliance with HIPAA. Answer telephone calls, schedule resident follow-up appointments and outside services, and coordinate transportation for medical visits. Assist with lab, x-ray, and therapy coordination, take resident photos, and update information as needed. Maintain organized nursing stations and forms while supporting residents, families, and staff. Requires CNA certification and strong computer skills in a fast-paced long-term care environment.
Required Qualifications
- CNA Certification per state requirements
- High school diploma or equivalent
- Strong organizational skills and attention to detail
- Computer, email, and data-entry skills
- Ability to manage multiple priorities in a fast-paced environment
- Excellent communication and customer service skills
- Ability to maintain confidentiality of resident information
- Compliant with the state's Health Care Worker Background Check requirements with respect to criminal history
Desired Qualifications
- Experience working in a long-term care, skilled nursing, rehabilitation, or hospital setting
- Previous medical records, HIM, or Unit/Ward Clerk experience
