Job Summary
Oversee daily clinical and administrative operations of the Subacute Unit, supervising and mentoring RNs, LPNs, and CNAs. Ensure high-quality resident care through effective care planning, medication management, and treatment oversight while monitoring documentation for accuracy and regulatory compliance. Investigate resident incidents, accidents, complaints, and grievances, ensuring timely documentation and follow-up. Collaborate with physicians, therapy, MDS, infection prevention, and social services to promote positive resident outcomes. Coordinate admissions, readmissions, and discharge planning to support continuity of care and reduce hospital readmissions. Participate in Quality Assurance and Performance Improvement (QAPI) activities and administer corrective action in accordance with facility policies. Maintain compliance with all federal, state, and local regulations while preparing for and participating in surveys and inspections.
Required Qualifications
- Current, active Registered Nurse (RN) license in the State of New Jersey
- Minimum of 2 years of nursing leadership or supervisory experience in a skilled nursing, rehabilitation, or long-term care setting
- At least 2 years of experience in subacute, rehabilitative, restorative, or post-acute nursing
- Strong clinical assessment, leadership, organizational, and communication skills
- Knowledge of state and federal long-term care regulations
- Ability to lead a multidisciplinary team in a fast-paced environment
