Job Summary
Assure effective, outcome-focused nursing care through the nursing process, including identifying needs, developing individualized care plans, and facilitating discharge planning. Supervise nurses, care specialists, and other personnel while implementing plans consistently with a focus on resident-centered outcomes. Deliver optimum resident care and comfort by demonstrating current nursing practices and collaborating with physicians and interdisciplinary teams. Requires current LPN licensure, CPR certification, and geriatric experience.
Required Qualifications
- current license to practice as a Licensed Practical Nurse (LPN) in practicing state
- current CPR certification
- Recent clinical experience, education, or specialty skills specific to geriatrics
- capable of maintaining regular attendance
