Job Summary
Assure effective, outcome-focused nursing care delivery through the nursing process and develop individualized care plans in collaboration with residents and the interdisciplinary team. Implement these plans consistently while delivering optimum resident comfort, and supervise Nurses, Care Specialists, and other personnel. Identify needs for discharge planning and follow-up throughout the length of stay, working with physicians and health care professionals to share relevant information for changing care plans. Maintain regular attendance and utilize current nursing practices to ensure resident-centered outcomes.
Required Qualifications
- current license to practice as a Licensed Practical Nurse (LPN) in practicing state
- current CPR certification
- Recent clinical experience, education, or specialty skills specific to geriatrics
- capable of maintaining regular attendance
