Job Summary
Assure effective, outcome-focused nursing care is delivered by identifying resident needs and providing input for discharge planning throughout length of stay. Develop individualized plans of care in collaboration with residents, families, and the interdisciplinary team, then implement them consistently and cost-efficiently. Supervise nurses, care specialists, and other personnel, including other RNs, while maintaining optimum resident comfort through current nursing practices. Requires current LPN licensure, CPR certification, and geriatric experience. Full benefits package includes excellent pay, health plans, 401(k), and education reimbursement up to $7,500 annually.
Required Qualifications
- current license to practice as a Licensed Practical Nurse (LPN) in practicing state
- current CPR certification
- Recent clinical experience, education, or specialty skills specific to geriatrics
- capable of maintaining regular attendance
