Job Summary
Assure effective, outcome-focused nursing care is delivered through the nursing process by identifying resident needs and providing input for discharge planning. Develop individualized care plans in collaboration with residents, families, and the interdisciplinary team, then implement them consistently and cost-efficiently. Supervise nurses, care specialists, and other personnel while working with physicians to share information relevant to changing care plans. Maintain optimum resident care and comfort by demonstrating current nursing practices. Requires a current LPN license, CPR certification, and geriatric experience.
Required Qualifications
- current license to practice as a Licensed Practical Nurse (LPN) in practicing state
- current CPR certification
- Recent clinical experience, education, or specialty skills specific to geriatrics
- capable of maintaining regular attendance
