Job Summary
Unit Care Coordinator role for an RN on a post-acute care unit. Responsible for supervising and coordinating patient care through interaction with patients, families, nursing staff, and other team members. Leads daily functions of unit RNs and CNAs, administers medications as ordered, coordinates care plans, ensures compliance with Life Care standards and applicable laws/regulations, and responds to emergencies with leadership and professionalism. Requires nursing licensure, nursing degree, and strong knowledge of post-acute care practices; patient rights, safety and sanitation. English literacy required.
Required Qualifications
- Nursing diploma (associate’s or bachelor’s degree in nursing)
- Active Registered Nurse (RN) license in good standing
- One year geriatric nursing experience preferred
- Advanced knowledge in field of practice
- Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility
- Read, write, speak, and understand the English language
