Job Summary
Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function. Administer, evaluate, and interpret tests to determine functional goals and outcomes, providing written evaluations and plans of care. Deliver physical therapy services complying with prescribed physician orders while maintaining discharge plans and educating patients and caretakers on home exercise regimens. Coordinate services with the healthcare team, physicians, and nurses, and supervise other personnel as applicable. Complete OASIS assessments where allowed by regulation and document effectively to prevent technical or clinical denials. Maintain effective communication with staff, healthcare team members, and families, serving as a resource consultant for clinical staff.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
