Deliver physical therapy services complying with prescribed physician orders while maintaining discharge plans and educating patients and caretakers on home exercise regimens. Coordinate services with the healthcare team, physicians, and nurses, and supervise other personnel as applicable. Complete OASIS assessments where allowed by regulation and document effectively to prevent technical or clinical denials. Maintain effective communication with staff, healthcare team members, and families, serving as a resource consultant for clinical staff.
