Job Summary
Deliver one-on-one, skilled nursing care in the home, conducting comprehensive assessments and translating findings into patient-centered care plans. Provide interventions including wound care, infusions, catheter care, medication management, and chronic disease management tailored to individual patient goals. Educate patients and caregivers on symptom and medication management while monitoring progress to prevent avoidable hospitalizations. Coordinate care across providers and complete timely, accurate documentation to support quality outcomes. Schedule Monday through Friday full-time visits with shared on-call rotation in the Prior Lake, Lakeville, and surrounding Minnesota territory. Requires an active RN license, one year of clinical experience, CPR certification, and reliable transportation.
Required Qualifications
- Holds an active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
- Is patient‐focused, organized, and motivated by delivering high-quality care
- Has a current driver's license
- Has reliable transportation
- Has auto insurance
- Is CPR certified
- Is able to obtain CPR certification
