Job Summary
Conduct evaluation, coordination, and administration of social work services to patients and families in Williamson, Davidson, and surrounding counties. Assist clients in understanding social and emotional factors related to health problems and develop plans of care based on clinical diagnosis and physician input. Arrange linkage to community medical and health resources, routinely review client progress, and ensure use of appropriate terminology. Investigate client or employee concerns, instruct families and caregivers on optimal functioning levels, and coordinate agency services by orienting other caregivers. Maintain accurate client data, initiate service plan meetings, and provide input to the Medicare Director regarding policy formulation. Ensure timely completion of documentation and transfer of medical data to physicians and therapists. Requires current state license, Master's degree in Social Work, one year medical social work experience, and valid CPR.
