Job Summary
Deliver one-on-one skilled nursing care in the home, including wound care, infusions, medication management, and chronic disease management. Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, translating findings into patient-centered care plans. Educate patients and caregivers on symptom and medication management while monitoring progress to prevent hospitalizations. Coordinate care across providers and complete timely, accurate documentation to support quality outcomes. Serve as the primary clinical partner for patients and families in the Citronelle, Wilmer, and North Mobile County territory. Monday through Friday full-time visits with a shared on-call rotation.
Required Qualifications
- Holds an active Registered Nurse (RN) license in the state of practice
- at least one (1) year of clinical nursing experience
- Is patient‐focused, organized, and motivated by delivering high-quality care
- Has a current driver's license
- Has reliable transportation
- Has auto insurance
- Is CPR certified
- Is able to obtain CPR certification
