Job Summary
Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, translating findings into patient-centered care plans. Provide skilled nursing interventions including wound care, infusions, catheter care, medication management, post-operative care, and chronic disease management tailored to patient goals. Serve as the primary clinical partner by educating and empowering patients and caregivers to manage symptoms and daily care independently. Monitor patient progress closely, identify changes in condition early, and communicate effectively with physicians and the interdisciplinary team to adjust plans and prevent avoidable hospitalizations. Coordinate care across providers to ensure continuity and complete timely, accurate documentation reflecting the full clinical picture. This part-time role covers Monday through Friday for Blount, Jefferson, and St Clair Counties areas.
Required Qualifications
- Holds an active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
- Is patient-focused
- Is organized
- Is motivated by delivering high-quality care
- Has a current driver's license
- Has reliable transportation
- Has auto insurance
- Is CPR certified or able to obtain certification
