Assess, teach, and deliver physical therapy interventions directed by the Physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels while administering tests to determine goals and outcomes. Provide written evaluations defining needs, progress, and expected outcomes, ensuring documentation complies with physician orders and payer standards to prevent denials. Collaborate with interdisciplinary team members, physicians, and families to optimize patient well-being, modify treatment plans, and coordinate discharge care including home exercise education. Supervise personnel such as PTAs and HHAs as applicable. Complete OASIS assessments where permitted by state regulations.
