Administer, evaluate, and interpret tests to determine functional goals and outcomes, then provide written evaluations defining needs, progress, and expected results. Deliver physical therapy services complying with physician orders while maintaining plans of care through timely documentation and modifying goals as needed. Collaborate with physicians, nurses, and caretakers for discharge planning and education on home exercise plans.
