Job Summary
Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current function for patients recuperating at home. Administer, evaluate, and interpret tests to define functional goals, outcomes, and discharge care plans while delivering services per physician orders. Maintain documentation of client performance, modify goals, and collaborate with physicians, nurses, and families to ensure effective communication and minimal supervision. Supervise other personnel including PTAs and HHAs where applicable, and complete OASIS assessments per state regulations.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
- Valid Drivers License
Desired Qualifications
- Experience in Home Health Therapy
- completion of OASIS documentation
