Job Summary
Deliver one-on-one, skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate patients and caregivers on symptom and medication management while monitoring progress to prevent hospitalizations. Coordinate care across providers and complete accurate documentation supporting quality and compliance. Work full-time daytime hours Monday through Friday with participation in a team shared on-call rotation. Requires an active RN license, one year of clinical experience, and CPR certification.
Required Qualifications
- Holds an active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
- Is patient-focused
- Is organized
- Is motivated by delivering high-quality care
- Has a current driver's license
- Has reliable transportation
- Has auto insurance
- Is CPR certified or able to obtain certification
