Develop a plan of care through physician orders and nursing assessment to include medical interventions and measurable goals. Educate clients and family members based on specific needs while properly orienting and training primary caregivers. Coordinate the continuum of hands-on client care through documentation and timely communication with physicians and other caregivers. Provide care in patients' homes using skills such as phone triage, wound care, infusions, and catheter care. Schedule: Monday - Friday full-time visits with shared on-call rotation. Requires an active RN license, valid CPR and driver's license. Preferred qualifications include Medicare Skilled Nursing experience and one year of RN experience in a health care setting.
