Job Summary

Develop a plan of care through physician orders, client input, and nursing assessment to include medical interventions and measurable goals. Educate clients and family members based on specific needs while properly orienting and training primary caregivers. Provide skilled nursing care in patients' homes using skills such as phone triage, wound care, infusions, and catheter care. Coordinate the continuum of hands-on client care through documentation and timely communication with the client's physician and other caregivers. Adhere to confidentiality standards and professional boundaries while documenting care in accordance with the developed plan. Monday through Friday part-time visits with a 7-day rotation.

Required Qualifications

  • An active RN License in the state of application
  • Valid CPR
  • Drivers License
  • Proof of automobile insurance
  • 1-year RN experience in a health care setting

Desired Qualifications

  • Medicare Skilled Nursing experience
  • Basic understanding of Oasis
