Job Summary
Conduct internal case management, evaluate nursing talent, and oversee patient transitions from facility to home through start-of-care assessments and re-evaluations. Review physicians' orders, update care plans, and assist in selecting, training, and validating new clinical staff while promoting clinical expertise. Collaborate with operations and business development teams to ensure adherence to state, federal, and local regulations. Lead field-based visits to mentor staff in patient homes, requiring travel to designated territories with reliable transportation and a valid driver's license.
Required Qualifications
- Must have good standing license (RN) in the state in which the clinician will practice
- Must have at least 1 year of hands-on nursing experience
- Must be able to travel to patients' homes in designated territory
- Must have reliable transportation, valid driver's license, and pass MVR check
- Current CPR certification from AHA or ARC
- Must be willing to supervise caregivers providing care to infants, children, adolescents and/or adults
