Job Summary
Conduct pain management and symptom control for hospice patients per the plan of care established by the interdisciplinary team and attending physician. Perform regularly scheduled and after-hour home visits to assess total patient and family needs, documenting assessments, interventions, goals, and outcomes. Coordinate total care under the Director's supervision, collaborate on clinical plan revisions, and ensure continuity between facilities and providers. Supervise Hospice Aides and Licensed Practical Nurses while facilitating service delivery in long-term care settings. Participate in on-call rotations for weekend and holiday coverage, monitor patient appropriateness for hospice services, and complete required documentation for certification.
Required Qualifications
- An active RN License in the state of application
- Valid CPR
- Monday- Friday with rotating on call
Desired Qualifications
- Medicare Skilled Nursing experience
- 1-year RN experience in a hospice setting
