Job Summary
Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, translating findings into patient-centered care plans. Deliver individualized, one-on-one nursing care including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate patients and caregivers on symptom and medication management while monitoring progress to prevent avoidable hospitalizations. Coordinate care across providers to ensure continuity and complete timely, accurate documentation supporting quality outcomes. Visit Monday through Friday with an on-call weekend rotation covering Sioux City and surrounding areas.
Required Qualifications
- Holds an active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
- Is patient‐focused, organized, and motivated by delivering high-quality care
- Has a current driver's license
- Has reliable transportation
- Has auto insurance
- Is CPR certified
- Is able to obtain CPR certification
