Job Summary
Develop and oversee care plans from admission to discharge while conducting in-home supervisory visits and providing direct care as needed. Train, mentor, and evaluate clinical staff, participate in hiring and performance management, and ensure regulatory compliance with standards such as CHAP. Monitor service quality, implement improvements, maintain equipment and medical supplies, and participate in an on-call rotation. Collaborate with care teams and external partners to support a positive, growth-oriented team environment in varied home settings. Requires an active RN license, valid driver's license, and ability to lift up to 40 lbs.
Required Qualifications
- RN License (active in applicable state)
- Associate's, Diploma, or Bachelor's in Nursing
- Valid driver's license and acceptable MVR
- Clinical experience per state/federal guidelines
- Ability to lift up to 40 lbs
- Walk, bend, kneel, and climb stairs
- Work in varied home settings
- Potential exposure to allergens and infectious diseases
