Job Summary
Coordinate care for assigned clients from admission to discharge by establishing goal-directed care plans with comprehensive ongoing assessments. Perform on-site supervisory visits to evaluate clients, families, environments, and clinical caregivers while ensuring proper operation of patient care equipment and supplies. Provide direct client care as needed and offer ongoing supervision, orientation, training, and evaluation of field staff. Manage expectations for internal and external customers, analyze data to identify service improvements, and obtain re-authorizations for continued care. Participate in after-hours client care calls and adhere to QA plans to promote safe, quality care. Schedule is Monday through Friday, 8am to 5pm, with occasional rotating on-call for evening or weekend emergencies.
Required Qualifications
- Must have good standing license (RN) in the state in which the clinician will practice
- Must have at least 2 years of hands-on nursing experience as an RN
- Must be able to travel to patients' homes in designated territory
- Must have reliable transportation, valid driver's license, and pass MVR check
- Current CPR certification from AHA or ARC
- Must be willing to supervise nurses providing in home skilled nursing care to infants, children, adolescents and/or adults
- Must be able to speak, write, read and understand English
- Must be able to lift 50 pounds
- Must be able to sufficiently reposition patients and move equipment without assistance
- Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
- Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
- Possible exposure to blood, bodily fluids and infectious diseases
