Job Summary
Conduct functional, psychosocial, and environmental safety assessments during home visits to determine enrollee eligibility and establish individual plans of care. Independently manage care for long-term patients while collaborating with interdisciplinary teams and communicating with diverse caregivers and providers. Apply knowledge of Medicare, Medicaid, and NYS DOH standards to ensure safe discharge and home-based treatment. Full-time position with reimbursement bonuses for extra visits; multiple openings in Dutchess County, Ulster, Orange, Rockland, Sullivan, and Putnam. Bilingual BSN graduates encouraged to apply.
Required Qualifications
- Current NYS RN license and registration
- Associates degree required
- Bachelor of Nursing degree preferred
Desired Qualifications
- Bilingual BSN new graduates encouraged to apply
- One year of experience in Homecare, acute care or community health nursing preferred
- Working knowledge of Medicare, Medicaid and NYS DOH standards and regulations
- Bilingual preferred
