Job Summary
Engage clients, families, and schools in collaborative relationships to enhance service completion and positive outcomes. Work with families, courts, and community partners to develop and implement service plans that facilitate regular school attendance. Maintain awareness of community resources to facilitate client access to necessary support services. Collect and monitor school data and client satisfaction materials in prescribed formats. Communicate with DHS, the Court, and school personnel regarding service status and outcomes for RTC-involved students. Accompany children and families through Truancy Court proceedings and submit all mandated documentation and statistical reports. Advocate for appropriate care placements while adhering to agency and regulatory guidelines. Participate in interdisciplinary functions and meetings. Full-time role with occasional evening shifts; travel within Philadelphia required. Valid driver's license needed.
Required Qualifications
- Valid driver's license required
- Valid driver's license required and driving record must meet CORA standards
- Bachelor's degree in social work, human services or related discipline
- Demonstrated case management experience
- Outstanding communication and interpersonal skills; ability to establish and maintain positive, productive working relationships internally and with external partners (e.g., court & school personnel, families, community partners, etc.)
- Ability to effectively manage assigned caseload variability
- Proficient with Office 365 applications
- Must be able to travel within assigned areas of the city
- Work schedule may include occasional evenings
Desired Qualifications
- Knowledge of substances of abuse and their effects preferred
