Job Summary
The Tribal WrapAround Care Coordinator provides care coordination case management services to high-risk individuals and families experiencing homelessness or at risk of homelessness. Responsibilities include maintaining a caseload, assembling a Tribal WrapAround Team, coordinating meetings and developing individualized Plans of Care, monitoring service quality, and linking clients to support services. The role requires a Bachelor's degree in relevant fields, at least one year of experience, and strong interpersonal and crisis management skills. The position also entails collaboration with various system partners to empower clients toward self-sufficiency.
Required Qualifications
- Bachelors Degree in Social Work, Sociology, Criminal Justice, Human Services or related field is required
- At least one year of relevant experience and a demonstrated track record in a similar related area
- Excellent crisis de-escalation and communication skills
- Must possess a valid California Driver's License with a clean driving record.
Desired Qualifications
- Masters degree in Social Work, Sociology, Criminal Justice, Human Services or related field preferred
- Extensive knowledge and experience working with Native American communities, culture, customs and cultural law.
