Job Summary
Oversees tribal health navigation to connect Native communities with healthcare services, supports care delivery and health equity in the Columbia River Gorge. Builds authentic relationships with Native communities, coordinates patient-centered care, provides health and safety group education, connects Tribal members with local resources, travels to outreach locations, and supports Community Health Worker certification. Requires strong interpersonal skills, cultural competency, travel readiness, driving privileges, and the ability to work with interdisciplinary clinical teams. Primary work site is Hood River clinic with travel via company vehicle; includes occasional evenings/weekends.
Required Qualifications
- High School Diploma or equivalent
- Minimum two years of experience working directly with Tribal communities and/or Native American community-based organizations
- Minimum of two years of experience in a health-related, community health, public health, or social service field
- Strong interpersonal and communication skills
- Commitment to culturally and linguistically appropriate, patient- and family-centered care coordination
- HIPAA compliance
- Ability to travel frequently within the Columbia River Gorge
- Valid driver’s license, clean driving and insurance record, minimum of 2 years driving experience
- Ability to meet organizational driving and background-check requirements, including operation of the Wy-Kan-Ush Pum Mobile Unit
