Triage RN, Primary Care

Conduct telephone triage, direct patient care, and care coordination for outpatient primary services. Perform wound care, injections, vaccinations, and flu clinic support while managing post-ED follow-up calls, scheduling, and chronic disease education. Administer medications, perform IV therapy, and maintain accurate records while collaborating with providers in a collaborative care team model. Uphold infection control protocols and ensure patient safety within a steady-volume practice seeing 130 patients daily. Full-time day shift from 8:30 AM to 5:00 PM.
