Triage RN

Triage all patients seeking same-day services, organize provider schedules based on medical and behavioral health acuity, and answer the triage phone to coordinate with hospitals and community partners. Develop individualized patient education plans, evaluate chronic disease management effectiveness, and ensure accurate documentation in the electronic health record. Maintain sensitivity with diverse populations, report safety concerns immediately, and complete incident reports per policy. Assist with preventative health programs, UDS initiatives, and new administrative projects as assigned by the Nursing Director. Maintain general office skills and communicate departmental activities to staff and committees. This full-time, 40-hour role requires an active Utah RN license and supports quality improvement initiatives to improve patient outcomes.
