Job Summary
Manage clinical triage and care management duties for the SMG administration team during the 8:00 AM to 5:00 PM, Monday-Friday shift. Coordinate patient intake and population health activities while utilizing EMR systems to document care plans. Maintain strong communication and critical thinking skills to support ambulatory care and primary care workflows. Requires a South Carolina-licensed RN with at least three years of clinical nursing experience and an accredited nursing degree.
Required Qualifications
- Graduate of an accredited school of nursing
- Licensed with South Carolina State Board of Nursing as a Registered Nurse (RN)
- Strong communication and critical thinking skills
Desired Qualifications
- Minimum of three (3) years clinical nursing experience
- Ambulatory care, primary care, care management, clinical triage, or population health experience
- EMR proficiency
