Job Summary
Independently provide telephone triage nursing services for Assisted Living and Hospice patients, accurately documenting symptoms, assessments, and advice while following established protocols. Follow up on high-risk situations with families or staff, analyze data trends to improve service outcomes, and design workflows for quality improvement. Provide high-level nursing support for patient care questions at assigned sites and efficiently document in multiple electronic health records. Maintain high clinical standards per the Nurse Practice Act and Ecumen policies. Work every 3rd weekend from 3pm to 11pm. Target pay is $36.00–$41.00 per hour. Required Qualifications
- Licensed as a Registered Nurse in MN
- Maintain licensure or obtain within 60 days of hire
- 1 year of care management experience in home care or hospice
- Strong clinical and relational skills
- Ability to analyze situation and implement solutions
- Ability to handle multiple priorities, ambiguity, frequent change and high stress situations
- Ability to communicate effectively in both verbal and written formats
- Proficient with Microsoft Office Suite
- Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations

- Shift availability 3pm-11pm
