Job Summary
Independently provide professional nursing services over the telephone for Assisted Living and Hospice support, accurately documenting symptoms, assessments, advice, and caller responses. Follow up on higher-risk situations with families or staff while analyzing data trends to improve services and implement quality improvement solutions. Maintain high clinical standards by efficiently documenting in multiple electronic health records and adhering to all safety policies including HIPAA and Resident's Bill of Rights. Requires an active Minnesota RN license, one year of care management experience, and the ability to handle high-stress ambiguity. Schedule every third weekend (Saturday & Sunday) from 7am to 3pm with remote work flexibility.
Required Qualifications
- Must be licensed as a Registered Nurse in MN
- Must maintain licensure or obtain within 60 days of hire
- 1 year of care management experience in home care or hospice
- Strong clinical and relational skills
- Ability to analyze situation and implement solutions
- Ability to handle multiple priorities, ambiguity, frequent change and high stress situations
- Ability to communicate effectively in both verbal and written formats
- Proficient with Microsoft Office Suite
- Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations
- Must have an active RN License in the state of Minnesota
- Must be available for weekend shifts
- Must be available for 7am-3pm shifts
- Must be available for Every 3rd weekend
