Job Summary
Triage Nurse responsible for interviewing and triaging patients to determine level of care, collecting and interpreting patient data with providers, documenting in the electronic health record after each encounter, initiating diagnostic and therapeutic measures per standing protocols, and providing health counseling on medications and adherence. Works within DVCH’s ambulatory care centers to support patient care, with collaboration to educate patients and caregivers (including bilingual Spanish/English support). Key activities include conducting triage via in-person and virtual modalities, interfacing with the Clinical Pharmacist for patient education, and maintaining high-quality, efficient clinical care across Urgent/Primary/Emergency contexts. Must hold PA RN license, graduated from an accredited nursing program, and CPR certified. Desirable experience includes medical/surgical, primary/urgent/ambulatory care, with strong communication, collaboration, and multi-line phone skills.
Required Qualifications
- Graduation from accredited school of nursing
- Current PA license as a Registered Nurse
- CPR Certification required
